I hereby voluntarily consent to the provision and processing of personal data according to Regulation (EU) 2016/679 in the scope of data contained in this form, namely in particular name, surname, email address, and telephone number by the controller, being the company TAYLLORCOX s.r.o. with registered office at Na Florenci 1055/35, New Town, 110 00 Prague 1, Czech Republic (hereinafter as "controller").

I provide my consent for the purposes of processing accredited courses, certifications, electronic communication, namely the provision of information by electronic mail or telephone, sending business messages through electronic means according to Act No. 480/2004 Coll., on certain services of the information society, in the current version.

Personal data will be processed by the controller manually and automatically.

I hereby acknowledge that the controller is obliged to provide me, upon my request, with information about the processing of my personal data, its purpose, about the personal data that are the subject of processing, including information about their source, nature of any automated processing, and about the recipients of personal data, namely those to whom my personal data have been made available.

I further acknowledge that I have the right to request from the controller an explanation of the processing of my personal data in case I discover or believe that this processing is not being carried out in a proper manner, and further the right to request from the controller a remedy of this situation consisting in particular in blocking, making corrections, supplementing or deleting personal data.

I provide my consent voluntarily and acknowledge that I have the right to withdraw it at any time. This consent is given for a period of 60 months from its granting.
